Please print out, complete, and return the registration form. Symposium registrations will be accepted on a first come, first-served basis. You will receive confirmation of your registration. In order for you to participate in the North Coast tour following the symposium, you must be registered for the conference. You will be notified if a waiting list becomes necessary. Registration fees must be received by the registration deadline of July 9, 2004. Registration fees are non-refundable.
