Κύριε Ἰησοῦ Χριστέ βοήθει τῷ σῷ δούλῳ Λέωντα.
Lord Jesus Christ, help your servant Leo.
Commentary
It is likely that the two letters missing at the beginning of the second line are ΧΕ, the abbreviation for Χριστέ.
Note the change in case in the invocation from the dative to the accusative in which the owner's name is rendered.
