Inscription in five lines, lower line lost. Border of dots.

Translation

Θεοτόκε βοήθει τῇ σῇ δούλῃ Εὐδοκίᾳ μοναχῇ καὶ δούλῃ τῆς Τριάδος.

Theotokos, help your servant Eudokia, nun and servant of the Trinity.
