Translation
Θεοτόκε βοήθει τῷ σῷ δούλῳ Θεοφάνει ἁμαρτωλῷ δούλῳ τῶν δούλων Χριστοῦ.
Theotokos, help your servant Theophanes, sinner, servant of the servants of Christ.

Commentary
In his unpublished catalogue to the Shaw collection, Laurent mentions that the ending of the formula found on the reverse presents a small but noteworthy change, Θεοῦ in place of Χριστοῦ, which is commonly found in bishops' signatures. See Laurent, "Mélanges d'épigraphie," 438.
