Translation
Ἀρχιστράτηγε βοήθει τῷ σῷ δούλυ σοῦ Κωνσταντίνῳ πρωτοσπαθαρίῳ.
Commander of the heavenly hoast, help your servant Constantine protospatharios.

Note the confused lettering in the final words of the invocation in the fourth line of the reverse.
