Translation
Θεοτόκε βοήθει τῷ σῷ οἰκέτῃ Θωμᾷ πρότῳ.
Theotokos, help your servant, the protos Thomas.

Commentary
The word protos seems to be written across the top of the monogram in the upper two angles using the upper arm of the cruciform in between, to read πρότῳ, while in the other three arms of the monogram are Θωμᾶ.
