Translation

Θεοτόκε βοήθει τῷ σῷ δούλῳ Κοσμᾷ ἀσηκρῆτις.

Mother of God, help your servant Kosmas, asekretis.

Commentary

The name and office are the most plausible, but it is possible that the office was protasekretis.
