Translation

ἅγιε βοήθει τῷ σῷ δούλῳ Θεωδώτω δοῦλος Χρηστοῦ.

Saint, help your servant Theodotos, servant of Christ.

Commentary

Note that the word δοῦλος is given in the nominative case instead of the expected dative.
