Translation

Κύριε βοήθει τῷ σῷ δούλῳ Κυρηακῷ πρωτοκεντάρχῳ.

Lord, help your servant Kyriakos protokentarchos.

Commentary

Note the abbreviation  on the penultimate line of the reverse; normally standing for καὶ, it is here used phonetically to represent κε in the word προτοκένταρχος.
