Θεοτόκε βοήθει τῷ σῷ δούλῳ Βασιλείῳ μωναχῷ τῷ Ἀνιώτῃ (?).

Commentary
The restitution of the name Ἀνιώτῃ, although reasonable, is not certain. The first letter of the family name may be rounded, and may thus perhaps be Φθιώτῃ (Phthiotes) or something of the like.
