St. Theodore standing, holding a spear and shield. Inscription in columns on either side: |θ|ε|οδ|ορ|οσ|ο|,Τ, : ὁ ἅ(γιος) Θεόδορος ὁ Στ(ρατηλά)τ(ης). Border of dots.

Inscription of five lines (beginning with a cross which because of damage cannot be seen). Border of dots.
