Translation
Κύριε βοήθει τῷ σο δούλῳ Ἰωάννῃ πρημηκηρήο καὶ ἐπὶ τοῦ κοιτῶνως.

Commentary
Note the mistaken use of the abbreviation for καὶ (second letter of the third line on the reverse).
