Translation

Θεοτόκε βοήθει τῷ σῷ δούλῳ Κωνσταντίνῳ πατρικίῳ, ὑπάτῳ, κριτῇ καὶ κατεπάνω Μεσοποταμίας.

Mother of God, help your servant Constantine, patrikios, judge, and katepano of Mesopotamia.
