Translation
Θεοτόκε (or Κύριε) βοήθει τῷ σῷ δούλῳ Θεοφάνῳ δούλο Χρηστοῦ.
Theotokos, help your servant Theophanes, servant of Christ.
Commentary
Corrosion of upper letters of the monogram makes it unclear whether the monogram reads Θεοτόκε βοήθει or Κύριε βοήθει.
