Translation
Κύριε βοήθει τὸν σὸν δούλον Μιχαὴλ σπαθαρίῳ τὸν Που..λον.

Commentary
The family name is uncertain. There is space at the beginning of the final line on the reverse for two letters. The upper portion of an angled element followed by two vertical elements suggests ΚΗ, suggesting a family name of Poukeles or Poukelos.
