Θεοτόκε βοήθει τῷ σῷ δούλῳ Βαανίῳ πατρικίῳ [ἐκ Θεοῦ].
Theotokos, help your servant Vaanios patrikios [by the grace of God].

Commentary
There is clearly something above the reverse block monogram; from a parallel specimen (Zacos-Veglery, no. 3231), Zacos shows that it is likely ΕΘΥ (ἐκ Θεοῦ), though it is entirely illegible on this seal. Additionally, other valid readings of the reverse monogram are Άβανίῳ, Ἰοβιανῷ, and Ἰουβιανῷ.
