Bust of the Mother of God holding the Child in her left arm. Traces of sigla at right (note that the letters are reversed): Υ̅Θ: [Μ(ήτη)ρ] Θ(εο)ῦ. Within a border of dots, remains of a circular inscription:

ΣΔ,

[Κ(ύρι)ε β(οή)θ(ει) τῷ] σῷ δ(ούλῳ)

Κύριε βοήθει τῷ σῷ δούλῳ Ἰωάννῃ χαρτουλαρίῳ καὶ ἐπισκεπτίτῃ.
