Translation

Θεοτόκε βοήθει τῷ σῷ δόλου Συμεὼν τῷ Σαραντινο.

Mother of God, help your servant Symeon Sarantinos.

Commentary

Note the unusual ligature of ΑΝ in the last line of the reverse. We also keep the mistakenly rendered δόλου which the engraver meant to give as δούλο.
