Translation
Θεοτόκε βοήθει τῷ δούλῳ σου Εὐφημίῳ (or Εὐθημἰῳ) ἀπὸ ἐπάρχων.
Mother of God, help your servant Euphemios (or Euthymios) apo eparchon.
Commentary
The uncertainty of the first letter in the second line of the reverse inscription makes it difficult to determine whether the owner's name was Euthymios or Euphemios.
